Dermatologen sollten ihre Rolle bei der Behandlung von Hautzellentumoren auf hohem Niveau behalten

Shannon C. Trotter, DO, FAOCD, a certified dermatologist working at Dermatologists of the Central States in Ohio, discusses immunotherapy options for the treatment of advanced cutaneous squamous cell carcinoma (cSCC) and why dermatologists should remain involved in the care process even after patients are referred to oncology. In an interview with HCPLive, Trotter emphasizes that the care of patients living with locally advanced or metastatic cSCC is a matter that must continue even after they are referred to oncology, and that dermatologists should play an active role in this process. In the interview, Trotter discusses immunotherapy options for advanced cSCC, as well as the expected Medicare cuts and the future of artificial intelligence in skin cancer care. At the Fall Clinical Dermatology Conference in Las Vegas, Nevada, on October 10, Trotter co-presented a session titled “Seminar-In-Depth: Deep Dive on Cutaneous Squamous Cell Carcinoma and the Latest in Immunotherapy” with Todd E. Schlesinger, MD.[^1] Trotter noted that approximately 95% of cSCC cases are treatable, but that approximately 5% become more challenging to manage. She highlighted three approved immune checkpoint inhibitors for locally advanced or metastatic cSCC: pembrolizumab (Keytruda), cemiplimab (Libtayo), and cosibelimab (Unloxcyt). In particular, research on formulations to be injected with cemiplimab could play a role in the clinic setting for dermatologists. “They may seem far away from dermatologists, but in fact, we are the quarterback in skin cancer. Whether it’s on the skin or we feel a lymph node, we may be the first to detect it. So even if our patients are referred to oncology, we must remain involved in the care process,” said Trotter.

Trotter also evaluated the anticipated Medicare cuts in terms of patient access. Older adults who are more affected by squamous cell carcinoma may experience delays in diagnosis and treatment. With early detection, the survival rate for melanoma is approximately 100%; Trotter added that the dermatology community has signed a petition against the cuts. Looking ahead, Trotter highlighted augmented intelligence, which combines artificial intelligence with physician reasoning, as a significant opportunity in both melanoma and non-melanoma squamous cell carcinoma. New diagnostic tools may enable earlier detection. Gene expression profiling can provide prognostic information, either alone or in combination with artificial intelligence, and may guide treatment decisions in the future.
